American Cancer Society/Illinois Division -- $25,000
For the Patient Navigation Services program in Lake County: provides information about cancer to persons diagnosed with this disease; transportation to and from treatment; lodging for patients and families during treatment; support services; help with completing Medicaid and prescription assistance applications; advice on improving personal appearance; information about clinical drug trials; and referrals
to other cancer-related organizations and community resources.

Community Youth Network -- $20,000
For the Living in Families Effectively (LIFE) Program: an intensive mental health program for families that are at high-risk for child abuse and neglect.

Independence Center -- $35,000
To help purchase a computerized information management system designed to gather information such as client demographic data, mental health treatment plans and outcomes, service utilization, referrals, and also to comply with new Illinois Department of Human Services reporting requirements.

Lake County Sponsors -- $30,000
For a comprehensive healthcare needs assessment; research into best practices of skilled-care and assisted-living services and facilities; health services and facility recommendations that are financially viable; and advocacy for high-quality health services for residents of the new Winchester House in Libertyville.

Lake Forest Hospital -- $35,000
For the CareCoach: a mobile health clinic that provides health education, disease screenings, immunizations, school and sports physicals, and medical consultations to uninsured and underserved Lake County residents in their communities.

NorthPointe Resources -- $25,000
For the Nursing Services Program: provides acute, intermittent and ongoing medical care and treatment to persons with intellectual, developmental disabilities, and/or chronic mental health issues who live in NorthPointe Resources’ 24 community homes.

Rosalind Franklin University of Medicine and Science -- $25,000
For mental health services provided at the Healthy Families Clinic in Waukegan: including diagnostic evaluations and individual and family therapy.

Rosalind Franklin University of Medicine and Science -- $75,000
For the Physician Assistant Scholarship Program: a new scholarship program designed to increase the number of Physician Assistants practicing in underserved areas of Lake County. 

United Way of Lake County -- $10,000
For FindHelpLakeCounty.org: an interactive, online, searchable database that connects individuals and families to health, education, social, recreational and other community resources.

Waukegan Township Senior Services/Park Place Senior Center -- $15,000
For the Senior Health and Wellness Program: provides onsite health workshops and disease screenings, and works directly with seniors to ensure they get the health services they need.

Zacharias Sexual Abuse Center -- $40,000
For the Child, Adult and Family Counseling Program: specialized mental health services designed to help clients heal and learn how to cope with the trauma of their sexual assault experiences.
