Grants Approved at the November 24, 2008 Board Meeting
Total amount awarded: $350,000

Catholic Charities -- $25,000
For HIV/AIDS medical case management services provided by Catholic Charities at the Lake County Health Department's Belvidere Medical Building.

HealthReach -- $100,000
For medical services provided primarily to uninsured Lake County residents.

Lake County Health Department -- $100,000
For specialty medical care and diagnostic testing services for uninsured patients with acute or chronic medical conditions.

PADS Crisis Services -- $50,000
To help fund the salary of a full-time Social Worker who provides clinical assessments; medical case management; counseling; support and life-skills groups; health education; and ongoing monitoring of Safe Haven residents with histories of chronic homelessness and serious mental illness or substance abuse.

YouthBuild Lake County -- $25,000
For mental health and substance abuse prevention services for program participants.

YWCA Lake County -- $50,000
For community health education and disease screenings for breast cancer, HIV/AIDS, cardiovascular disease, and sexually transmitted infections for low-income women and girls.
