For transportation services designed to help low-income and home-bound seniors access medical appointments; community education about the importance of exercise for preventing falls and maintaining overall good health; and a survey to assess the seniors’ needs for medical and mental health screenings and how best to help seniors access them

Evanston Northwestern Healthcare — $25,000
For home-based health education, medical and mental health referrals, and follow-up for participants enrolled in a health disparities study

For home-based counseling services for low-income children enrolled in Waukegan public elementary schools and their families.

For community health education and referrals to services

PADS Crisis Services — $25,000
For medical and mental health services provided to residents of Safe Haven, PADS’ housing facility for adults with histories of chronic homelessness and serious mental illness

St. Martin de Porres High School — $25,000
For health and wellness education for freshmen and sophomore students

For onsite medical services for persons with severe and persistent mental illness

Waukegan Public Schools — $25,000
For a business development plan to create a new school-based health center in the Waukegan High School

YouthBuild Lake County — $25,000
For mental health services for program participants

YWCA Lake County — $35,000
For community health education about breast cancer, HIV/AIDS, cardiovascular disease, and sexually transmitted infections for low-income women and girls

Zion Benton Children’s Service — $25,000
For free and reduced-price dental services and oral health education for low-income patients as well as community oral health education
